1. You may obtain the relevant application form, from the Cashier’s Office on the Administrative block.
2. Click here to download the application form . On completion of the downloaded form, the applicant must submit the form along with the other supporting documents and the respective application processing cost to the address below.
3. Fill the form out online (using this page). Please note that you are still required to submit the other supporting documents and the respective application processing cost to the address below. The application processing cost will vary depending on the program you are applying for.
